Clinical Case Report: Tricuspid Valve Replacement Surgery for Severe Tricuspid Valve Stenosis and Dilated Right Atrium Following Post Tetralogy of Fallot (TOF) Repair in a Pediatric Girl Patient from Saudi Arabia

Findings: As per the shared reports, the young girl was diagnosed with congenital heart disease Tetralogy of Fallot (TOF), for which she subsequently underwent corrective (TOF repair) surgery.

Following the surgery, current findings indicate that the right atrium is dilated, with the interatrial septum (IAS) bowing toward the left side. Additionally, there is a severely stenotic tricuspid valve orifice, consistent with severe tricuspid valve stenosis.

Tricuspid stenosis (TS) is a rare heart valve disorder involving the narrowing of the tricuspid valve, which restricts blood flow from the right atrium to the right ventricle

We will need to confirm the diagnosis by doing some tests upon the patient’s arrival.

Investigations needed to be done: The patient may need to undergo a detailed physical evaluation, including CAG, ECG, ECHO, Blood tests, and the entire routine test to confirm the above diagnosis.

Possible line of treatment: The final line of treatment will be concluded after the investigation outcomes. But in all possibilities, she will require Tricuspid Valve replacement.

Tricuspid valve replacement surgery is a high-risk, complex open-heart procedure performed to replace a previously repaired or replaced tricuspid valve that has failed or become severely diseased again. It involves replacing the dysfunctional valve with a new mechanical or biological prosthesis, often necessitated by severe, persistent regurgitation.
