Clinical Case Report: Endoscopic Retrograde Cholangiopancreatography (ERCP), and Chemotherapy for Bile Duct Cancer (Cholangiocarcinoma) in a 64 Year Old Female Patient from Yemen

Patient is a case of suspected / Known case of cholangiocarcinoma

Cholangiocarcinoma, also known as bile duct cancer, is a rare and aggressive malignancy that forms in the thin tubes carrying digestive bile from the liver and gallbladder to the small intestine. It is generally categorized by where it occurs: intrahepatic (inside the liver) or extrahepatic (outside the liver, such as hilar or distal). 

Investigations Needed to confirm the diagnosis – 

1. CA 19-9 for tumour confirmation
2. PET CT for staging Evaluation
3. ERCP for stent placement and tissue biopsy.
4. Molecular Testing for tumour profiling
5. Other routine tests

Possible Lines of Treatment –

Step 1 – ERCP as suggested above in diagnosis.

ERCP (Endoscopic Retrograde Cholangiopancreatography) is a specialized medical procedure that combines upper gastrointestinal endoscopy and X-rays to diagnose and clearing blockages and placing stents to drain excessive fluids such as bilirubin (biliary drainage).

Step 2 – Once the biliary drainage is achieved, we plan for chemotherapy.

Chemotherapy is a type of cancer treatment that uses one or more anti-cancer drugs as part of a standardized chemotherapy regimen. Chemotherapy may be given with a curative intent, or it may aim to prolong life or to reduce symptoms.

Chemotherapy cycles will be once every two or three weeks. Thus, he may need 15-18 weeks to finish the chemotherapy.

Stay in India – Patient may require 2-3 days for initial evaluations on an OPD basis and he will take 1 day for ERCP. The chemotherapy may take up to 15 weeks and will start after 1 week of ERCP. The total stay in India will be between 15-18 weeks.

On Arrival: Patient and attendants will be received at the airport by our Representative. Apart from the medical and nursing team, one Case Manager will take care of all the needs. We shall also assist them with reasonably charged accommodation near the hospital. You may prepare USD 25-45 per day towards accommodation in the guest house after discharge from the hospital.
