Treatment of Sickle Cell Disease – Sample Opinion

For treatment of Sickle Cell Disease, the patient needs to undergo Allogenic bone marrow transplant. 

Th Allogenic Bone Marrow / Stem Cell Transplant uses healthy blood stem cells from a matching donor’s body to replace bone marrow that’s not working properly. For this, a Healthy and matching donor is needed. This is usually a DNA matched sibling.

After the complete evaluations and HLA Reports, If it comes out to be 100% matched (from a biological sibling), then the doctors will go for 100% HLA Matched BMT.

But in Sickle Cell disease to find a healthy sibling whose Bone Marrow does not have sickle cell traits is sometimes difficult. So, we will need to find a sibling who does not have the trait.

In case a matching donor is not available or a healthy sibling with a sickle cell trait is not available, then we may take donation from half match donor such as either of the parents or less matched siblings (who are sickle cell free but may not be a complete match). This is called Haplo Identical BMT. Usually someone from the family (parents or siblings) will be 50% HLA Matched.
