Overview

An Autologous Bone Marrow Transplant (ABMT), also known as an Autologous Stem Cell Transplant or Bone Marrow Transplant is an advanced medical procedure in which a patient’s own healthy blood-forming stem cells are collected, preserved, and later reinfused after the patient receives high-dose chemotherapy. The treatment is designed to rescue and restore healthy bone marrow function after intensive therapy aimed at eliminating cancer cells.

Purpose of the Procedure

The primary goal of an Autologous Bone Marrow Transplant is to allow patients to tolerate much higher, more effective doses of chemotherapy than their bone marrow could normally survive. While high-dose chemotherapy destroys the cancer, it also destroys the blood-making cells in the bone marrow; reinfusing the saved stem cells acts as a safety net to rebuild the system.

Procedure Process

1. Pre-Transplant Assessment

The patient undergoes rigorous medical testing—including extensive blood work, imaging (PET/CT scans), bone marrow biopsies, and cardiac/lung function tests—to ensure their organs are strong enough to safely tolerate the intensive treatment.

2. Stem Cell Mobilization and Collection

Patients receive injections of growth factors (and sometimes low-dose chemotherapy) to stimulate the bone marrow to produce an abundance of stem cells and push them into the bloodstream. These cells are then collected via a machine through a process called apheresis (similar to a specialized blood donation) and are safely cryopreserved (frozen) until needed.

3. High-Dose Conditioning Chemotherapy

The patient is admitted to the hospital or specialized transplant unit to receive intensive, high-dose chemotherapy. This phase targets any remaining cancer cells in the body, but temporarily wipes out the patient’s existing bone marrow and immune system.

4. Stem Cell Reinfusion (The “Transplant”)

A day or two after the chemotherapy conditioning ends, the frozen stem cells are thawed and reinfused back into the patient through a central venous catheter, a painless process very similar to a standard blood transfusion.

5. Engraftment and Recovery

Over the next 2 to 3 weeks, the infused stem cells travel through the bloodstream to the bone marrow cavities, nestle in, and begin producing new blood cells. This critical milestone is called engraftment. During this phase, patients are kept in a highly protective, clean environment and given supportive care (blood transfusions, antibiotics, and nutritional support) until their immune systems recover.

Temporary Side Effects: High-dose chemotherapy causes temporary side effects including severe fatigue, nausea, hair loss, loss of appetite, and painful mouth sores (mucositis).

Specialized Infrastructure Required: The procedure must be performed at a dedicated transplant center with specialized medical infrastructure and expert nursing care.

Variable Recovery Time: While initial blood count recovery (engraftment) takes a few weeks, full energy levels and complete immune system maturity can take several months to a year to return to normal.
