Clinical Case Report: Multimodal Management of Keratinizing Epidermoid Carcinoma (Squamous Cell Carcinoma) of the Eye via Surgical Resection, or Chemotherapy / Targeted Therapy / Immunotherapy in a 65 Year Old Patient from Uganda

As per the shared reports, the patient has been diagnosed with Keratinizing Epidermoid Carcinoma involving the Conjunctiva.

Keratinizing Epidermoid Carcinoma (also known as squamous cell carcinoma) involving the conjunctiva is the most frequent malignant tumor of the ocular surface, arising from the squamous epithelial cells of the conjunctiva.

The conjunctiva is a thin, transparent mucous membrane that lines the inner eyelids and covers the white part of the eyeball (sclera), extending to the edge of the cornea. We will need to perform some investigations upon patient arrival to confirm the diagnosis.

Investigations required: The patient would require a detailed physical and clinical examination which includes imaging tests like MRI, PET CT, Biopsy, blood tests, molecular study and NGS study and other routine investigations.

Treatment plan: The final treatment plan will be decided only after the outcome of complete investigations and reviewing the patient in person.

If the disease is localized, she will need surgery.

If the disease is advanced and has metastasized, patient will need chemotherapy with target therapy or immunotherapy.

Depending upon investigative outcomes of the molecular study and NGS study, if any mutation is detected, the doctors will add Targeted therapy. If the mutation is not detected, the doctors may need to add Immunotherapy.

Surgery for Epidermoid tumor of eye involves surgical excision to remove the tumor, including its capsule to prevent recurrence this may involve an incision in the eyebrow to approach the tumor or an orbital approach depending upon tumor size and location.

Chemotherapy is a type of cancer treatment that uses one or more anti-cancer drugs as part of a standardized chemotherapy regimen. Chemotherapy may be given with a curative intent, or it may aim to prolong life or to reduce symptoms.

Targeted therapy is a cancer treatment that uses drugs to target specific molecules on cancer cells. These molecules control how cancer cells grow, divide, and spread. 

Immunotherapy is a treatment that uses the body’s immune system to fight cancer, infections, and other diseases. It can also be used to treat allergies.

The cycles of Chemotherapy will run once every 2 weeks. Thus, it may take between 12-15 weeks to finish the treatment.
