Clinical Case Report: TMJ Arthroplasty Surgery in Maxillofacial Case of 36 Year Old Patient from Uzbekistan TMJ Ankylosis with Severe Restricted Mouth Opening & Retruded Mandible

Findings: According to the shared report, the patient sustained an injury in early childhood, at a critical developmental stage. This trauma led to abnormal maxillofacial growth with a retruded mandible and misaligned dentition. These contributed to significant functional limitations, including severely restricted mouth opening. This condition can be associated with improper development of his TMJ (TMJ Ankylosis).

 Temporo-mandibular joint ankylosis is a restrictive condition where the temporomandibular joint fuses due to bony or fibrous tissue growth, severely limiting jaw movement. Primarily caused by trauma or infections, it results in the inability to open the mouth, causing severe chewing, speech, and oral hygiene issues.

We will need to confirm this diagnosis upon his arrival.

Investigations needed: The patient would need to undergo a detailed physical and clinical examination, which includes maxillofacial Panoramic X-rays, CBCT, MRI, mandibular deviation test, Maximal Incisal Opening (MIO) Measurement, blood test, and other routine examinations.

Possible line of treatment: The final treatment plan will be determined after complete investigation outcomes. However, considering the clinical history, the patient is likely to require TMJ Arthroplasty.

TMJ arthroplasty is an open-joint surgical procedure that reshapes, repairs, or replaces the TMJ to treat joint ankylosis (fusion) or chronic dysfunction unresponsive to conservative care. It involves preauricular incisions to access the joint to remove bone spurs, reposition the disc, and replace the joint, usually resulting in significantly improved jaw function.
