Duodenal Papilla Mass Whipple Procedure

Medical Case Report: A 61-year-old Patient from Uzbekistan with a Pancreatic Head and Ampullary Mass Lesion Recommended for Further Evaluation and Possible Whipple Procedure

Diagnosis: In the shared reports, the findings are suggestive of a mass lesion in the region of the pancreatic head and the Major Duodenal Papilla. Further, the patient has a history of duodenoscopy, stenting of the Wirsung duct and common bile duct.

Pancreatic cancer is a cancer that develops in the pancreas, a gland that produces digestive juices and hormones.

Duodenal papilla cancer, also called ampullary cancer, is a rare cancer located in the ampulla of Vater, where the bile and pancreatic ducts join to empty into the small intestine.

We will need to confirm with more investigations whether the case is operable or not. We will also need to see how advanced the disease is in other organs and accordingly plan the treatment.  

Evaluations: The investigations – PET CT, Biopsy, Ultrasound, CT scan, CBC, LFT, KFT, ultrasound, blood investigation, and other related investigations to confirm the above diagnosis.

Treatment Plan: The final treatment plan will be determined after the investigations are completed. Once the diagnosis is confirmed, the treatment plan can only be decided based on the outcomes of the PET scan and biopsy reports.

If the pancreatic cancer is confirmed with limited disease spread and we find the case operable. then the patient may require a Whipple Procedure.

If it is found that the disease has advanced and it is not wise to operate, then the patient may require Chemotherapy.

Whipple procedure is a major surgical operation most often performed to remove cancerous tumours from the head of the pancreas. A Whipple surgery, or pancreaticoduodenectomy, is a procedure to remove the head of the pancreas, the first part of the small intestine (duodenum), the gallbladder, and a portion of the bile duct, most commonly to treat tumours in the pancreatic head.

Estimated Cost:

The estimated cost for detailed evaluation will be approx. USD 1000-1200 on OPD basis.

In case he is not found to be a candidate for surgery and only chemo is offered, the cost of chemo may come between $5000 and $7500 for 5-6 cycles of chemotherapy. The chemo will be administered on a day care basis.
